Presentation: 50F, 2m lethargy, jaundice, itch; deranged LFTs
Diagnosis- anti-smooth muscle antibodies
Management: steroids (good response)

Subacute tiredness, itchy
30-60yr old female
Examination
Jaundice
Scratch marks
Hepatomegaly
Investigations
Deranged LFTs: ALT > AST (both raised)
Antibodies
Anti-smooth muscle antibody
ANA
Anti- LKM
Diagnostic criteria: (3/3)
Exclude other causes of acute hepatitis
Clinical signs + antibodies
Liver biopsy if uncertainty
Classification:
Type 1: anti-smooth muscle antibody
Type 2: anti-LKM antibody
Acute
Corticosteroids
Usually a good response to steroids
Long term: maintain remission
Azathioprine (requires TPMT)
