Pathogen: Brucellae- bacteria, gram negative coccobacilli (short rods)
Also called Mediterranean fever
Presentation: fever, dry cough, arthralgia in a European animal farmer
Management: rifampicin and doxycycline 6 weeks
History
Farmer, cut leg with livestock
Presents with fevers and joint pains
Examination
Small joint polyarthritis
Lymphadenopathy, splenomegaly, hepatomegaly +- rash
Normal observations
Investigations
Normal FBC, urea & electrolytes, LFTs and CRP
Doxycycline 6 weeks +
Rifampicin OR
Gentamicin
