Exam Cram

• Pathogen: Brucellae- bacteria, gram negative coccobacilli (short rods)

• Also called Mediterranean fever

• Presentation: fever, dry cough, arthralgia in a European animal farmer

• Management: rifampicin and doxycycline 6 weeks

• Farmer, cut leg with livestock

• Presents with fevers and joint pains

Examination

• Small joint polyarthritis

• Lymphadenopathy, splenomegaly, hepatomegaly +- rash

• Normal observations

Investigations

• Normal FBC, urea & electrolytes, LFTs and CRP

Diagnosis

Management

1. Doxycycline 6 weeks +

   1. Rifampicin OR

   2. Gentamicin
